Receiving Treatment at SLM

1. Determine Your Eligibility

To receive treatment services from SLM, you must be at least 18 years old. You must consent in writing to voluntary participation in methadone/buprenorphine treatment and complete a physical exam by an SLM staff physician. SLM gives priority to pregnant women and applicants who test positive for HIV.

You are also required to furnish documentation that you were dependent on heroin or other opiates during a one-year period prior to admission. An SLM staff member will explain this requirement during the prescreening telephone interview. This one-year dependency is not required for pregnant women or HIV-positive patients.

2. Schedule Screening Interview

If you would like to begin treatment at SLM, please call or visit our facility to schedule an interview. During your interview, a staff member will explain available SLM services and fees, as well as program rules and requirements.

3. Receive Medical Check

After your screening interview, you will need a complete medical work-up at SLM. The work-up includes a medical history and a physical exam with blood work and urine samples. It also includes tuberculosis (TB) and HIV and Hepatitis C testing are available upon request. Pregnant patients will be referred for prenatal care and an SLM follow-up.

4. Bring Photo ID

You will need two ID’s, one of which must be a photo ID when you are admitted to SLM.

Transferring to Another Facility

SLM will transfer you to another program for continued maintenance, detoxification, or another type of treatment if you request a transfer, or if the SLM treatment team determines that the transfer is in the best interest of your health and safety. All transfer requests are acted upon within one week. You may also be referred to a hospital for detoxification. If the transfer is denied, we will document the reason why in your record.
